for Memberships, Gift Cards, and Packages

Membership Policy


  1. Membership Benefits: Memberships provide discounted rates on services and exclusive promotions. Specific benefits vary by membership tier.
  2. Billing & Payment: Membership fees are auto-billed monthly. A valid payment method must be on file.
  3. Cancellation: Members may cancel with at least 7 days' notice before the next billing cycle. No refunds for partial months.
  4. Pausing Membership: Memberships may be paused once per year for up to 2 months. Requests must be made in writing.
  5. Non-Transferable: Memberships are non-transferable and may only be used by the registered individual.

Gift Card Policy


  1. Redemption: Gift cards can be used for services or retail purchases at Evolve EMS Therapy
  2. Expiration: Gift cards do not expire but cannot be redeemed for cash.
  3. Non-Refundable: All gift card sales are final. Lost or stolen cards cannot be replaced.
  4. Transferability: Gift cards may be transferred to another person with written approval.

Package Policy


  1. Expiration: Packages expire 6 months from the date of purchase unless otherwise stated.
  2. Non-Refundable: All package sales are final. No refunds, but remaining sessions may be transferred to another person with approval.
  3. Usage: Packages cannot be shared unless explicitly stated in the package terms.
  4. Scheduling, Cancellations & No-Shows:


Appointments must be scheduled in advance.

A 24-hour notice is required for cancellations or rescheduling.

Failure to cancel or reschedule within 24 hours will result in the deduction of one package session as a no-show fee.

By purchasing a membership, gift card, or package, you agree to these policies. Policies are subject to change at Evolve EMS Therapy’s discretion.
